Parents & Students will need to be together to complete these forms. To gain access, students will need to be signed into their MHS gmail account.
This form must be printed and filled out completely. Turn in completed form & money to Mrs. Reece in the office.
This form must be completed EACH school year to be in compliance for us to refuse the military access to your student's records.
Enrollment is voluntary but necessary if you plan to make use of.
Your Advantage Card must be enrolled every year. This helps our school get money to spend towards school supplies.
